Hospital Discharge Project
Jigsaw Support manages Tameside’s Hospital Discharge Project, to provide a smooth transition of care from hospital to home. The service is primarily based at Tameside Hospital, but is open to anyone who is a Tameside resident, not just Jigsaw residents.
This service allows clients to continue support in their own home to prevent admission/ readmission into the hospital, allowing clients to access services such as:
- Adult Social care
- Occupational therapy
- Drug/alcohol services
- Social prescribing
- Welfare benefits advice
- Motiv8
Support is continued for as long as needed until a client is referred for more extended support or through non-engagement.
